Polycystic Ovary Syndrome (PCOS): Why Is Diagnosis Still So Difficult?




This complex hormonal condition was first identified about 90 years ago, but there are still no FDA-approved medications to treat it.


After nearly a century of disagreements about what exactly defines this disorder and a lack of research, PCOS remains poorly understood. The symptoms vary so much that it is unlikely that a single drug will help all patients, said Dr. Heather Huddleston, a reproductive endocrinologist at the University of California, San Francisco, and director of the UCSF PCOS Clinic.


Polycystic ovary syndrome (PCOS) has been recognized for almost a century since its first description in the medical literature. However, despite its longevity in terms of clinical recognition, it remains one of the most difficult endocrine conditions to diagnose and treat.


This raises serious concerns in the realm of women's reproductive and overall health. We will explore why PCOS is so difficult to detect and treat, its possible causes, characteristics, and clinical significance, and current treatment approaches.



Challenges in Detecting and Diagnosing PCOS:


PCOS is characterized by a combination of symptoms including menstrual irregularities, hyperandrogenism (excess male hormones), and polycystic ovaries on ultrasound imaging. However, the lack of a uniform presentation of these symptoms and individual variability make diagnosis a challenge. Many women with PCOS do not receive a proper diagnosis for years, leading to inadequate or lack of treatment.





Causes and Contributing Factors:


Although the exact cause of PCOS is not fully understood, there is believed to be a complex interplay between genetic, hormonal, and environmental factors. Insulin resistance, where the body's cells do not respond properly to insulin, is a commonly associated factor. This can lead to elevated levels of insulin in the blood, which in turn increases androgen production in the ovaries.



Common Characteristics:


Polycystic ovary syndrome (PCOS) is a complex medical condition that can manifest in different ways in each individual.


  • Menstrual Irregularities: Women with PCOS often experience irregular or absent menstrual cycles. This can manifest as infrequent, prolonged, or very light menstrual periods.


  • Hyperandrogenism: Hyperandrogenism refers to elevated levels of male hormones (androgens), such as testosterone, in a woman's body. This can cause symptoms such as severe acne, excess facial or body hair (hirsutism), and hair loss on the scalp (androgenetic alopecia).


  • Polycystic Ovaries: On ultrasound images, the ovaries of women with PCOS often show multiple small cysts, which are immature follicles that have not released an egg during the menstrual cycle. Although the term "polycystic" suggests the presence of multiple cysts, not all women with PCOS exhibit this characteristic, and not all women with polycystic ovaries have PCOS.


  • Insulin Resistance: Insulin resistance is common in women with PCOS. This means that the body's cells do not respond properly to insulin, which can lead to elevated blood sugar levels and increased insulin production by the body.


  • Obesity or Difficulty Losing Weight: Women with PCOS have a higher risk of being overweight or obese, and they often find it more difficult to lose weight. Insulin resistance and hormonal imbalances can contribute to this issue.


  • Fertility-related Symptoms: Many women with PCOS experience difficulties conceiving due to ovulation problems. Infertility can be a significant concern for some women with PCOS

PCOS not only affects reproductive health but is also associated with an increased risk of developing other medical conditions, such as type 2 diabetes, cardiovascular disease, and endometrial cancer. Additionally, PCOS symptoms can have a significant impact on women's quality of life, affecting their emotional well-being and self-esteem.
